
When you book an appointment you will be sent a new client form to sign.
You will also be asked to start a 7 day food diary. If you are seeking help for gut issues it will also include a symptom diary.
It is also helpful to have any test results or blood tests available for your appointment.
You can choose to send the above back to me before your appointment or you can bring them to your appointment. We will go through them during your initial consultation.
Your initial appointment is up to 60 minutes and reviews are 30 minutes. During your initial consult a full assessment will be conducted including your medical history, lifestyle and dietary habits, areas of concern and any symptoms you are experiencing. We will also discuss your goals.
A treatment plan will then be developed for you and any required resources provided.
At your follow up review we will assess your progress and ongoing support or adjustments to your treatment plan will be made.
If you do not attend your appointment or cancel within 48 hours of the appointment, you will be charged 50% of the total cost of the appointment and if less than 24 hours or you do not attend, 100% of the cost of the appointment. If we are unable to fill your appointment.
